Application Form for Care Home Fees Shortfalls

It is recommended that this application form is downloaded, printed off and returned by normal mail with the following papers. Please complete this form in BLACK ink. The form can be accepted by email to k.wallis@officersassociation.org.uk but the following documents will still be required:

a. The Local Authority Financial Assessment and written advice of the Local Authority contribution to the funding.

b. A copy of the extant Power of Attorney document.

c. Written confirmation from the selected Care Home of their weekly fees.

d. Written confirmation of State retirement, Service and occupational pensions.

e. Written or photocopy details of capital holdings.

We cannot accept applications without the above listed documents.
